About

At the heart of our service is a deep commitment to advocating for every client we care for. We believe that exceptional care goes beyond meeting physical needs. This means being a constant voice for our clients' preferences, dignity, and wellbeing. This advocacy is built into every layer of our organisation, ensuring that our clients are truly seen, heard, and supported in every aspect of their care journey.

How do we do this?

We understand that needing care, whether suddenly or over time, can feel overwhelming, emotional, and confusing. Families are often faced with difficult decisions, complex options, and a lack of clear information. That’s why we make it our mission to take on the heavy lifting, removing the pressure from clients and families by guiding them every step of the way.

Here’s how we help:

  • Initial Consultation & Advice: Our team offers honest, expert advice on funding and care options, and what to expect, without pressure or obligation.

  • Care Planning Support: We guide families through the entire process of arranging care, ensuring all medical, emotional, and lifestyle needs are considered and then met.

  • Liaison with Medical Teams: We work directly with GPs, consultants, hospital discharge teams, and private clinics to coordinate care on your behalf.

  • Paperwork & Practicalities: We handle care agreements, documentation, medication records, rotas, and everything in between.

  • Availability & Reassurance: We’re available 24/7 to answer questions, offer reassurance, and provide informed input when choices feel difficult or unclear.

From the very first phone call, our goal is to make your journey easier, calmer, and completely supported.

Our nurse-led, office management team play a pivotal role in ensuring our clients’ health and wellbeing are continuously monitored, understood, and prioritised. We serve as a bridge between our clients, their families, and other health care professionals, ensuring the right care is delivered at the right time, and that clinical concerns are proactively addressed. From coordinating multidisciplinary input to facilitating access to private consultants or GPs, our team ensure that each client receives tailored medical support and this voice is central in all decision-making.

Our Client Liaison Nurses (CLNs) work closely with each client and their family to design bespoke care plans that reflect not just clinical needs, but lifestyle preferences, routines, and values. They maintain regular contact, adapt care packages as situations evolve, and ensure seamless communication across all involved parties.

Further to this, we advocate for our clients by equipping our care staff to also be their advocates. All of our care staff undergo in-depth training from our in-house Clinical Nurse Manager / Tutor, covering not only essential clinical skills, but also communication, compassion, safeguarding, and dignity in care. Our team is trained to notice and escalate concerns, to respect every client’s right to choice, and to always act in their best interest. Staff also receive ongoing updates, reflective practice opportunities, and supervisions and appraisals to ensure advocacy remains part of their everyday practice—not just a policy.

Upholding Dignity and Choice

We believe in empowering our clients to make decisions about their own care, no matter their age or health condition. Our team advocates for client rights, informed consent, and freedom of choice—ensuring everyone we support is treated with dignity, involved in care planning, and never made to feel like a passive recipient of services.

Family Partnership and Transparent Communication

We advocate not only for our clients, but with their families. We work in close partnership with loved ones, ensuring they feel heard, respected, and included. We provide updates, listen to concerns, and act on feedback swiftly. Families know they can trust us to represent their loved one’s interests with professionalism, compassion, and discretion.
